Abstract

Background: Diabetes mellitus, which is associated with elevated blood glucose, lipids and liver enzymes, may results in alterations in the production of inflammatory cytokines, including chitinase-3-like protein 1 (YKL-40) and high-sensitivity C-reactive protein (hs-CRP), which induce and promote complications of diabetes such as atherosclerosis.The aim of this study was to evaluate the serum level of chitinase-3 like 1 protein (YKL-40) and high-sensitive C-reactive protein (hs-CRP) in type 2 diabetes with coronary artery disease compared to diabetic and healthy control. Methods: This cross-sectional study was performed on 87 subjects in four groups, including: 23 samples as healthy control (Group 1), 22 patients with type 2 diabetes (Group 2), 20 patients with coronary artery disease (Group 3) and 22 patients with type 2 diabetes and coronary artery disease (Group 4). Serum Fasting blood sugar (FBS), Cholesterol, Triglyceride, Low density lipoprotein (LDL-C), High density lipoprotein (HDL-C), Aspartate aminotransferase (AST), Alanine aminotransferase (ALT), YKL-40 and hs-CRP were determined. Results: The mean serum levels of FBS (Group 1 with 2 and 4, Group 2 and 3, P = 0.001), Cholesterol (Group 1 and 3, P = 0.03), Triglyceride (Group 2 and 3,p=0.027), HDL-C (Group 1 with 3 and 4, P = 0.02, P = 0.01 respectively), ALT (Group 1 and 4, P = 0.03, Group 2 and 4, P = 0.02) and AST (Group 2 and 4, P = 0.009) were significantly different..The mean serum levels of YKL-40 were significantly difference in the group 1(4.81±1.27 ng/ml) and group 2 (15.52±4.61 ng/ml) (P=0.01), group 3 (19.2±2.75 ng/ml, P=0.017) and group 4 (16.1±4.17 ng/ml, P=0.04). Also, The mean serum levels of hs-CRP in the group 3 (4.49± 1.53 μg/ml) and group 4 (1.28 ± 0.43 μg/ml) was significantly difference (P= 0.028). Conclusion: Serum levels of YKL-40 are increased in people with type 2 diabetes and coronary artery disease. It is recommended to determine the serum level of this marker in these patients. © 2019, Tehran University of Medical Sciences. All rights reserved.
